Many companies selling acne treatment products warn you there is no immediate and fantastic solution to prevent or treat acne or rosacea. They also say "it is a trial and error process to find the right product that works for you."

And since they can't refrain from their own evil giggling, they might add: "you will be better off if you try our products to avoid being disturbed by the same worries that haunt so many adults and teens as acne leaves behind horrid scars".

Let’s just face it. Doctors, scientists, laymen and most everybody under the sun, have been for eons trying to understand what causes teen and adult acne breakouts, theorizing and disputing almost everything about it except its existence. The disputes about acne include its causation, histopathology, pathophysiology, the role of diet in acne flare-ups, and the best cure or treatment for acne pimples, zits, and acne cysts on the back, face and shoulders.

There is also disagreement about the many classifications schemes that so called "experts" come up with, to describe all types of acne. If those descriptions where right, you would have at least half the solution, for as you know the best way to start to solve a problem is by describing it correctly, is it not?

No doubt you have to do your own research on acne before it is too late. The sad news is you will find out that there are too many promising products. With these finding you can only ponder if they have any truth behind them. Is there really a chance they will prevent and heal your acne breakouts? Is this the best product for me?

Most products have been designed to either kill bacteria with harsh chemicals, dry out the skin to eliminate most of the oil produced by your own body, unclog pores by partially exfoliating some of the outer layer of your skin with natural or chemical acids, reduce sebum production with dangerous drugs, or a combination of these.

Is there anyone left we can trust?

Dr. Danne Montague-King, the man who zapped Brad Pitt's scars and made Gwyneth Paltrow even prettier, says "95 per cent of over-the-counter cosmetics don't actually do anything." And adds the reason may well be that "America is a very litigious country" and cosmetic companies "are not going to put anything totally active in skin care products because they don't actually want to make any changes because people can misconstrue changes in their skin as allergies". He goes on remembering: "When I was covered in acne, I knew things could be done beyond the conventional things that were being done. I've always felt that most physicians become lazy once they've finished medical school and just become puppets for the drug companies."

We can't say such statements are right to the point. After all the nonsense you know so many people pay for, you are probably not ready to buy any of those “clever” concoctions, be it “botanical”, “herbal”, “pharmaceutical” or the new fussy “cosmeceutical” category, are you not?

I will at least assume you wish not to fall prey to the same nonsense.

So, bear with me —and hold this in the back of your mind— while we search together for truth in the claims made by the many different treatments being advertised.

First, we’ll dismiss outright all those saying “this is a miracle in a bottle” and others alike. Our time and effort is better focused on the ones warning us, “not to get caught up in false promises, for acne can not be cured; only placed under certain control”.

What Is The Root Cause of Acne Not Addressed?

A root cause is an initiating cause of a causal chain which leads to an outcome or effect of interest. Commonly, root cause is used to describe the depth in the causal chain where an intervention could reasonably be implemented to change performance and prevent an undesirable outcome.

Most Acne & Rosacea Products Only Address The Symptoms:

• Bacterial Infection with harsh chemicals that produce irritation, redness, extreme dryness and free radicals that damage the cells and cause premature aging of skin, or antibiotics that have become ineffective as they induce the development of bacteria resistant to antibiotics, also a problem when more serious health issues need to be addressed.

"We are very excited about these findings because they may provide the basis for improving the treatment and management of this condition," said Dr. Richard Gallo, chief of the division of dermatology at the University of California-San Diego and lead investigator of the NRS-funded study. "By defining the process leading to the inflammation, new medications might be developed to block these effects."

Dr. Gallo explained that when the normal immune system is faced with any of a broad range of potential dangers -- such as sun exposure, emotional stress, heat and spicy foods, among many others -- receptors recognize potential threats and protect the body by prompting the production of protective substances that isolate and neutralize any harmful effects. With rosacea, however, these protective substances, like overzealous guards, turn the body on itself, leading to inflammation.

Using advanced mass spectrometry technology to analyze the biochemical composition of proteins in rosacea patients, the researchers discovered an abnormality in the production of protective molecules and in the overabundance of a substance that can spur dormant protective molecules into action.

"The next step is to test these findings through various therapeutic interventions," Dr. Gallo said. "As we gain a thorough understanding we can look for new medications that block this process in order to treat or prevent the inflammation associated with rosacea."

In inflammatory acne lesions, NF-κB signaling is activated (a transcription factor critical for up-regulation of many proinflammatory cytokine genes). As a consequence, inflammatory cytokine genes (eg. TNF-α and IL-1ß) are induced. These primary cytokines will propagate the inflammatory response by acting on endothelial cells to elaborate adhesion molecules (eg. ICAM-1) to facilitate recruitment of inflammatory cells into the skin. TNF-α and IL-1ß will also stimulate the production of secondary cytokines, such as IL-8, which can aid in the motion of inflammatory cells towards the increasing concentration of these particular chemicals. By working through their cell surface receptors, TNF-α and IL-1ß not only amplify the NF-κB signaling cascade, but also activate MAP kinases (mitogen-activated protein kinase) to stimulate AP-1 mediated gene transcription (another important transcription factor involved in inflammation). As a consequence of activator protein 1 (AP-1) activation (cJun induction), AP-1-driven matrix metalloproteinases (MMPs), a class of zinc-dependent protease enzymes, are elaborated by resident skin cells. Along with neutrophil collagenase and neutrophil elastase enzymes brought in by (PMNs) polymorphonuclear leukocyte inflammatory cells, they degrade extracellular matrix proteins such as collagen and elastin. This is followed by matrix synthesis and repair, which is imperfect. Most of the imperfections would leave clinically undetectable deficits in the organization or composition, or both, of the skin matrix. However, when they occur to a significant extent throughout time, accompanied by sustained procollagen synthesis, acne scarring becomes clinically visible. American Journal of Pathology. 2005. Now of course there are factors that "cause" hyperactivity of our glands, clogged pores, bacterial infection and the like. Stress is the hands-down number one cause of sebaceous glands hyperactivity that may turn out into acne inflammation. An there are other contributing factors as well, including picking and squeezing pimples, pressure or friction (head bands, caps etc), lifestyle, birth control pills, cosmetics, climate and diet, but stress is the Big Kahuna! Again, you can not blame your body for producing large amounts of sebum. The problem is mostly how it reacts to injury inflicted to the cells that line the sebum canals, when the canals become overcharged with sebum pressing to outflow to the surface —where it belongs—, or when they are not able to reach the skin surface because of clogged pores and pinched-off sebum glands.

Counteracting the Acne & Rosacea Triggers

Got the point? Modulating the inflammatory response is the recipe for success! Cool down guys (immune cells), past are the times when bacteria and other micro-organism ruled. Or is it just wishful thinking? Is it not true that we carry on and inside our body three times more bacteria than the number of cells our organism is made of?

Fact is our body is ingrained with the memories of past battles, fought to protect our system whenever damaged or injured and does not listen to just any argument made up by the pharmaceutical or cosmetic companies that sell anti acne products aiming to control bacteria.

During our evolutionary drift, injuries and wounds represented a serious threat to our organism, not only due to blood loss, but also due to tissue damage or infection from the invasion of foreign bodies such as dirt, splinters and bacteria. The adult wound healing mechanism that evolved to respond to this threat has two fundamental characteristics: First, there is a rapid and robust inflammatory response, with recruitment of activated macrophages, neutrophils and lymphocytes to the injured site; and second, there is a fibrotic "walling-off" response to isolate the foreign body, with liquefaction of adjacent tissue leading to abscess formation and scarring.

In fact acne lesions are in this respect similar to any skin injury. Our system responds with the same strong inflammatory knee-jerk to the sebum that is supposed to outflow to the surface of the skin but remains trapped within the hair follicles as if it where a foreign body; or when sebum production is exacerbated and the first flow of sebum through the previously empty duct creates shear forces of sufficient magnitude that injure the walls of the sebum canals or the pilosebaseous glands.

Inflammation exacerbates when acne bacteria (Propionibacterium acnes) and other deliriously happy bacteria always present on the surface of the skin (Staphylococcus aureus, Staph. epidermidis, diphtheroids, streptococci, Candida, etc) start to proliferate uncontrolled in skin punctured or broken. The problem is magnified if right there the deliciously rich feeding fatty sebum is plenty.

Is the inflammatory response still appropriate ?

Scar Less Healing

An abscess and its subsequent scar is not an evolutionarily optimized end point for today's injuries, Dr. Mark Ferguson, D.D.S., Ph.D., professor in the faculty of life sciences at the University of Manchester, England, says. The abscess and scarring response, with its massive inflammatory overdrive, is optimized for a very different type of skin lesion than those occurring nowadays where hygiene and clean faces and bodies rule. The abscess and scar is induced by this inappropriate inflammatory response...

Lesions that affect the cells lining the sebum canals, arising from a sudden overflow of sebum or pinched-off sebum glands, occur in an otherwise clean skin without dirt or contamination by foreign bodies. These lesions should therefore be ideal candidates for healing, without complications, by a regenerative wound healing mechanism rather than a scarring mechanism. And the outcome should be characterized by scar less healing.

The healing process inside the hair follicles should be very much like what our system accomplishes for fetal wounds which heal quickly by regenerating cells and leaving absolutely no trace behind.

Fortunately we have found, by chance and keen observation, a better model. One geared as a different evolutionary immune response and cell regeneration system, by a family of invertebrates (spineless animals). Abstract: Licochalcone-A, Polyol Soluble Licorice Extract P-U (now in the market as Licochalcone LR-15), prepared from the roots of Glycyrrhiza Inflata Batalin shows several activities such as inhibitory actions of testosterone 5 ALPHA-reductase, lipase and phospholipase A2, as well as androgen receptor antagonist, antimicrobial and SOD-like antioxidant actions, which relate to skin care, especially the suppression of acne formation and development.
